Certifcates of Insurance
Certficates are issued for the Period of September 1st through August 31st. Do not request a new one until the old one is about to expire. Download this Form, fill it out and e-mail it to Rich Reddington. When the request is for a town, indicate "All" on the form, rather than requesting for individual fields. This will cover you for all the town fields.| This year all registrations must be submitted in spreadsheet format. The new form which provides editing to assist in data entry. The form is available in MS Excel or OpenOffice format in English and Spanish. |
